Asthma

Asthma is a disease of diffuse airway inflammation caused by variety of triggering stimuli resulting in partially or completely reversible bronconstriction.

Epidemiology:
The prevalence of asthma seems to have increased since 1970s, and asthma now seems to affect 4 to 7% of people worldwide.

Prevalence is higher for people <18 yrs than for those 18 to 64 yr and is higher for males before puberty and in females after puberty.

Aetiology (cause)
Development of asthma is multifactorial and depends on interaction between multiple susceptibility genes and environment factors.
Evidence clearly implicates household (dust mites , cockroach, pet) and other environmental (pollen) allergens in disease development in older children and adults. Air pollution is not definitively linked to disease but can trigger exacerbations.
Asthma also has been linked to perinatal factors such as young maternal age, poor maternal nutrition, prematurity, low birth weight, and lack of breast feeding.
Signs and Symptoms:
Patients with mild intermittent or mild persistent asthma are typically asymptomatic between exacerbations. Those with more severe disease or those with exacerbations experience dyspnoea, chest tightness, audible wheezing, and coughing; coughing maybe the only symptoms in many patients.
Symptoms can worsen during sleep, often around 4 am. Many with more severe disease suffer nocturnal awakenings (nocturnal asthma)
